America Med Connect LLC — Emergency & Complication Acknowledgment

This acknowledgment explains the respective responsibilities of the Client, healthcare providers, emergency services, and America Med Connect LLC (“AMC”) if an urgent medical concern, complication, or unexpected change occurs before, during, or after medical travel. AMC provides non-clinical coordination and concierge support and is not an emergency medical service.
1. AMC Is Not an Emergency Medical Provider
AMC is not a hospital, clinic, physician, nurse, ambulance service, or emergency medical provider. AMC does not diagnose complications, determine whether symptoms are serious, prescribe treatment, provide medical clearance for travel, or replace the treating provider or local emergency services.
2. What the Client Should Do in an Emergency
If the Client experiences symptoms that may constitute a medical emergency, the Client should seek appropriate medical care immediately by calling local emergency services, going to an appropriate emergency facility, and/or contacting the treating provider according to provider instructions. The Client should not wait for AMC to determine whether emergency medical care is needed.
3. Provider Emergency Instructions
Before treatment, the Client should obtain or be given access to provider instructions covering after-hours contact information, symptoms requiring urgent evaluation, where to seek emergency care, medication instructions, postoperative concerns, follow-up appointments, travel restrictions, and when return travel is considered appropriate. AMC may help the Client locate or obtain these instructions but does not independently create or interpret them.
4. AMC Assistance During an Emergency or Complication
When reasonably possible, AMC may help contact the treating provider or facility, coordinate transportation, assist with interpreter services, contact an authorized companion or family member, help with lodging extensions or travel changes, and coordinate follow-up logistics. AMC’s involvement does not substitute for medical evaluation or treatment.
5. Emergency Transportation
Routine AMC-arranged transportation may not be appropriate for a medical emergency. When emergency medical transportation is needed, the decision regarding ambulance or other medically appropriate transportation should be made by emergency personnel or healthcare professionals.
6. Medical Complications
Medical or surgical treatment may involve complications that occur before discharge, during recovery, or after the Client leaves the treating facility. Possible consequences include additional medical evaluation, testing, procedures, medication, emergency treatment, hospitalization, extended recovery, additional follow-up, delayed travel, extended lodging, additional transportation, and additional medical or travel expenses.
7. Additional Costs
Unexpected medical or travel circumstances may create costs not included in the original estimate, including emergency care, hospitalization, diagnostic testing, medication, additional procedures, medical equipment, ambulance services, flight changes, extended lodging, transportation, interpreter services, and companion expenses. Unless AMC specifically agrees otherwise in writing, these costs are the Client’s responsibility.
8. Extended Stay
The treating healthcare provider may determine that the Client should remain at the destination longer than originally planned. AMC does not determine when the Client is medically cleared to travel. AMC may assist with non-clinical arrangements associated with an extended stay.
9. Return Travel
The treating healthcare provider determines whether medical restrictions or recommendations affect return travel. AMC may coordinate around those instructions but does not provide medical clearance.
10. Complications After Returning Home
Complications can develop after the Client returns home. Before leaving the treatment destination, the Client should know how to contact the treating provider, what follow-up is recommended, whether local follow-up is needed, what documentation to obtain, and what symptoms should prompt urgent evaluation according to provider instructions.
11. Local Follow-Up Care
AMC cannot guarantee that a healthcare provider in the Client’s home community will accept responsibility for postoperative or follow-up care performed by another provider. The Client is encouraged to discuss local follow-up needs with the treating provider before traveling.
12. Authorized Emergency Contact
The Client may authorize AMC to contact a designated emergency contact during an urgent event, complication, hospitalization, travel disruption, or other significant case issue. AMC may share only the coordination information reasonably necessary and authorized by the Client.
13. Companion Responsibilities
If the Client travels with a companion, the companion is not an AMC employee unless specifically stated otherwise. The treating provider should explain any medically necessary companion or caregiver requirements.
14. Communication With Providers
During an emergency or complication, the Client authorizes AMC to communicate with the treating provider or facility for limited non-clinical coordination purposes such as contact, scheduling, transportation, follow-up, travel changes, and interpreter assistance, subject to the Client’s privacy authorization and applicable law.
15. No Guarantee of Provider Availability
AMC cannot guarantee that a specific physician, hospital, specialist, ambulance, interpreter, or other service will be immediately available or that emergency treatment will be included in the original medical estimate.
16. Insurance
Travel insurance may exclude planned medical treatment, complications from elective procedures, pre-existing conditions, extended medical stays, or medical evacuation related to planned treatment. The Client is responsible for reviewing actual policy terms and exclusions. AMC does not guarantee coverage or reimbursement.
17. Changes to the Treatment Plan
An unexpected medical finding may cause the treating provider to change, delay, cancel, or replace the planned treatment, require additional testing or hospitalization, or require a different facility. These are clinical decisions made by healthcare professionals. AMC may coordinate resulting logistical changes.
18. Client Responsibility to Report Changes
The Client agrees to notify the healthcare provider and, when relevant to coordination, AMC of material changes such as new illness, hospitalization, medication changes, new diagnoses, recent surgery, significant injury, mobility changes, provider-imposed travel restrictions, or treatment-date changes.
19. Refusal of Medical Advice
If a licensed healthcare provider recommends emergency evaluation, postponement of travel, extended observation, additional treatment, or another medical precaution, the Client’s decision to disregard that recommendation is their own. AMC personnel should not advise the Client to disregard provider medical instructions and may decline to participate in logistical arrangements that directly conflict with documented safety instructions.
20. Client Acknowledgment
By signing or electronically accepting this acknowledgment, the Client confirms that they understand AMC is not an emergency medical provider, should seek appropriate medical care without waiting for AMC when an emergency may exist, understand medical providers make all clinical decisions, understand complications may create additional medical and travel expenses, understand treatment or recovery may require a longer stay, understand AMC does not provide travel clearance, and understand AMC does not guarantee emergency services, provider availability, medical outcomes, or insurance coverage.
