America Med Connect LLC — Client Service Agreement

This Client Service Agreement (“Agreement”) describes the services provided by America Med Connect LLC (“AMC”) and the responsibilities of the individual receiving those services (“Client”).

1. Purpose

AMC provides non-clinical medical travel coordination, concierge support, provider introductions, logistical assistance, and related services for individuals considering healthcare outside the United States.

2. AMC Is Not a Medical Provider

AMC is not a hospital, clinic, physician, dentist, surgeon, nurse, or other healthcare provider. AMC does not practice medicine, diagnose conditions, prescribe treatment, determine medical candidacy, or make medical decisions for the Client. All clinical decisions remain with appropriately licensed healthcare professionals.

3. AMC Services

  • Provider identification and introductions.
  • Consultation and scheduling coordination.
  • Organizing client questions and process information.
  • Guiding and facilitating the medical-records transfer process between the Client and healthcare providers.
  • Coordinating provider-supplied estimates and logistics.
  • Travel, lodging, transportation, interpreter, and non-clinical support coordination.
  • Follow-up communication and logistical coordination after treatment.

The exact services, fees, and exclusions may be documented separately in an invoice, service proposal, case summary, or other written communication accepted by the Client.

4. Provider Selection

AMC may identify or introduce providers based on available information, relationships, location, services, credentials, facilities, communication, pricing, logistics, and other relevant factors. Provider acceptance is not guaranteed. The final provider and treatment decision belongs to the Client.

5. Medical Records

The Client is responsible for obtaining or requesting records required for provider evaluation. AMC may guide and facilitate the medical-records transfer process but does not intend to serve as the permanent custodian or repository of the Client’s medical records. Whenever practical, records should move directly from the Client or existing healthcare provider to the receiving healthcare provider.

6. Provider Estimates and Charges

Medical prices and treatment estimates are established by healthcare providers and facilities, not AMC. Estimates may change because of additional testing, findings, procedures, medications, implants, complications, hospitalization, or other medically necessary services. AMC does not guarantee final provider charges.

7. AMC Service Fees

AMC fees are compensation for non-medical coordination, concierge, travel, or related services. Before collecting a fee, AMC will provide a written description of the service, price, payment method, due date, and applicable cancellation or refund terms. Unless stated otherwise in writing, AMC fees are separate from healthcare-provider charges.

8. Cancellations and Refunds

AMC service-fee cancellation and refund terms will be disclosed before payment is collected. Provider, hospital, airline, hotel, transportation, interpreter, and other third-party charges are governed by their own terms. AMC cannot guarantee a third-party refund.

9. International Medical Travel Risks

The Client understands that international medical travel may involve travel-document requirements, healthcare-system differences, language barriers, schedule changes, extended recovery, delayed return travel, additional lodging or transportation, medical complications, and difficulty obtaining follow-up care after returning home.

10. Insurance

AMC may encourage the Client to investigate appropriate travel insurance, trip protection, complication coverage, or related products. AMC does not guarantee availability, coverage, eligibility, reimbursement, or claim payment.

11. Emergencies and Complications

AMC is not an emergency medical service. If the Client experiences a possible medical emergency, the Client should contact local emergency services, go to an appropriate emergency facility, and/or contact the treating provider according to provider instructions. AMC may assist with non-clinical communication and logistics but does not determine symptom severity or provide treatment.

12. Client Responsibilities

  • Provide accurate and truthful information.
  • Disclose relevant medical information directly to healthcare providers as requested.
  • Review provider recommendations, estimates, and instructions.
  • Obtain necessary travel documentation.
  • Inform AMC promptly of material changes affecting coordination.
  • Maintain appropriate communication with AMC and providers.

13. Third Parties

Healthcare providers, hospitals, laboratories, pharmacies, airlines, hotels, transportation companies, interpreters, insurers, lenders, and other vendors are independent third parties unless expressly stated otherwise. AMC does not control their professional judgment, operations, schedules, pricing, policies, availability, or performance.

14. No Guarantee of Results

AMC does not guarantee medical candidacy, provider acceptance, treatment outcomes, recovery time, absence of complications, specific cost savings, final pricing, travel availability, appointment availability, financing approval, insurance reimbursement, or immigration approval.

15. Client Choice

The Client may ask questions, request information, seek another opinion, decline a provider, postpone or decline treatment, or discontinue AMC coordination subject to applicable payment and cancellation terms. AMC will not knowingly pressure a Client to undergo a medical procedure.

16. Communications

The Client authorizes AMC to communicate regarding the case through approved methods such as telephone, email, secure systems, videoconference, or approved messaging platforms.

17. Electronic Acceptance

This Agreement and related case documents may be provided, accepted, acknowledged, and signed electronically when legally permitted.

18. Complaints

Clients are encouraged to notify AMC promptly in writing at amc@americamedconnect.com about concerns involving communication, scheduling, billing, provider coordination, travel, privacy, vendor performance, safety, or other AMC services. Medical complaints should also be directed to the treating healthcare provider.

19. Limitation of AMC’s Role

Nothing in this Agreement creates a physician-patient, dentist-patient, nurse-patient, attorney-client, insurer-insured, or other licensed professional relationship between AMC and the Client.

20. Changes to the Case

Treatment schedules, provider availability, pricing, travel arrangements, and recovery requirements may change. Material changes should be confirmed in writing whenever reasonably practical.

21. Governing Law

This Agreement is intended to be governed by the laws of the State of South Carolina, except where another applicable law cannot lawfully be waived or displaced.

22. Entire Agreement

This Agreement, together with the AMC Cancellation & Refund Policy, the Privacy, Communication & Records Coordination Authorization, the Emergency & Complication Acknowledgment, applicable written service descriptions, invoices, provider estimates, treatment-decision documents, and other documents specifically incorporated into the Client’s case, governs AMC’s services.

23. Severability

If any provision is determined to be invalid or unenforceable, the remaining provisions will continue to apply to the fullest extent permitted by law.

24. Acknowledgment

By signing or electronically accepting this Agreement, the Client acknowledges that they have read it, understand AMC’s non-clinical role, understand that medical decisions are made with licensed healthcare providers, understand that AMC fees and provider charges may be separate, understand that international medical travel involves risks and possible additional expenses, and agree to the responsibilities described here.
